Truck driver dies after accident in Warsaw restaurant

The driver of the semi-trailer involved in the accident at the Texas Roadhouse in Warsaw on Thursday afternoon has died.

An off-duty Indiana State Trooper witnessed the accident and said the driver was slumped over.

The police officer used the automated external defibrillator (AED) provided by his department on the driver of the semi-trailer and was able to measure the pulse.

According to Warsaw police, resuscitation was performed immediately.

An AED device was also used during transport to the hospital, but attempts to resuscitate the driver ultimately failed.

The Kosciusko County Coroner pronounced 77-year-old Lloyd Dale Howard of Culver dead a short time later at Lutheran Kosciusko Hospital.

Culver was operating a grain truck and was on his way home at the time of the accident.

On Thursday, police responded to the Texas Roadhouse after several people reported seeing the driver of a semi-trailer slumped over the steering wheel drive off the road and into a ditch.

The truck then crossed an access road, struck a NIPSCO electrical box, causing transformer oil to leak into the parking lot, and finally struck the Texas Roadhouse.

Their report states that over a dozen employees were in the restaurant preparing for the day when the accident occurred. No one was injured. Damage was caused to two areas of the restaurant, which has not yet reopened.

The hazardous materials team was called because of the oil spill on the transformer.
