The Washington Metrorail Safety Commission (WMSC) has released more details about a communications error in June that could have led to an accident, WJLA reported.

On June 20, WMSC reported that a northbound Blue Line train loaded with passengers was given orders to proceed at up to 55 miles per hour as it approached a switch, but should have been ordered to stop completely because a Yellow Line train was also approaching the switch.

As the train approached the blue line, the driver saw a red signal and pulled the emergency brake.

The WMSC concluded that the cause of the failure was that employees had inadvertently disabled the automatic train protection system, which prevents collisions between trains, during routine testing. WJLA reported that the safety commission and Metro had agreed to stop conducting tests when passengers were on the train and to use only out-of-service sections of track.
